Support Hub
Shared Lives is a care and support service which offers an alternative personalised support for vulnerable adults. The support helps adults to live independently in their community with the support of a family and community network. People are matched with Carers based on shared interests, lifestyles, and sense of humour.
The aim is to help people achieve their own goals and outcomes with the support of their identified carer.
This could include:
Day support - Hours and days to meet the users needs and requirements
Respite - This could be single nights, weekends, short breaks or holiday stays.
Long term - Live in support with a shared lives family.
Activities are based around the individual and what they hope to achieve with their support.
Support is available for people with Mental Health issues, Learning Disabilities/Difficulties, Autism, Dementia, Physical Disabilities or Brain Injuries.
